Summary

Persist candidate-bound independent-review intent, semantic receipts, findings, uncertainty, and separate delivery status in the existing operation/artifact journal.
Require an explicit cold-review dispatcher when the approved verification policy requires independent review; bind the request to immutable candidate, Attempt, spec, policy, repository, base, and head subjects.
Link remediation to the latest finished Attempt, carry exact findings in immutable context, respect Program/authority attempt budgets, and pause owner decisions, comments, repeated or oscillating findings.
Preserve legacy Attempt digests/codec records and export review verdict/delivery plus remediation lineage separately.

Validation

make check: 1,452 passed, 85 skipped, 106 subtests passed.
make package-check: reproducible sdist/wheel, isolated installation, and CLI smoke passed.

Synthetic fixtures only; no dependency changes or live inference.
